Coaching Agreement

This agreement by and between . hereafter referred to as Coach and . hereafter referred to as Client is entered into this

SECTION 1: Guidelines

Coaching is an ongoing relationship between individuals who understand and agree to the following:

1. Coaching is not therapy or a substitute for therapy.

2. Coaching is for people who are basically healthy emotionally, capable of functioning effectively, and willing to do the work to make positive changes in their lives. It does not seek to heal or correct pathological disorders or dysfunctions.

3. Coaching is a confidential relationship which the participants agree to keep all information strictly confidential, except in those situations where such confidentiality would violate the law or cause harm to the Client or another person.

4. Coaching deals with current challenges and works forward. It does not seek to heal the past.

5. Coaching addresses issues that the person being coached (Client) wants to improve upon and provides avenues for better, more efficient and more effective ways of accomplishing goals. It is the Client’s responsibility to take the appropriate action to affect the desired outcome.

6. Issues may include, but are not limited to, personal development, emotional wellness, career or business development, relationship enhancement, lifestyle management, life balance, decision making, goal achievement, spiritual growth and any other agreed upon topic.

7. Coaching is an ongoing relationship that can last from one month to several months or years, depending on the scope of work.

8. In coaching groups, the coach and peer coaching group should agree to the length of the commitment at the beginning of the process.

9. Time commitments may be renewed or terminated at any time.

10. Coaching may include any or all of the following:

a. Questioning
b. Examining lifestyles and behaviors
c. Brainstorming
d. Values clarification
e. Goal-setting
f. Identifying plans of action
g. Assignment of specific, goal related study
h. Accountability
i. Agreements for behavioral changes

11. Coaching is most effective when all parties are honest and straightforward in their communication. These will be given by the coach and are expected from the Client.

12. Coaching assumes that each person in the relationship will act honestly, responsibly and ethically at all times.

SECTION 1: Guidelines

1. Coaching typically involves connecting weekly, semi-weekly, or semi-monthly over a predetermined amount of time.

2. Meetings may be face to face, by telephone, Skype or email.

3. Coaching sessions range from 30 minutes to 2 hours with an average of 60-90 minutes, depending on needs and circumstances.

4. Coaching session payment plans vary based on client. Packages require a deposit at the first session with an option to make payments from session to session. One-time coaching sessions, such as RAMP, Career Strategy or follow-up sessions, are payable at the time of the session or, upon agreement between Coach and Client, will be invoiced for payment 15 days after the session.

5. If coaching by phone or Skype, Client will call or connect at the agreed upon time.

6. Prior to the session, Client will complete a Getting Started Form and submit it to the Coach for review. This is to ensure efficiency and speed of progress during the session itself. The conversation may involve personal updates, progress summaries, identification of areas that need attention, the nature of the work to be done, work assignments, support, and guidance through exploration.

SECTION IIi: Coaching Agreement

Coach

1. Coach agrees to partner with Client to identify and achieve Client’s personal and/or professional goals.

2. During the time Coach and Client spend together in coaching sessions, Coach will devote his/her thoughts and energy to Client exclusively.

3. Between sessions, Coach will return Client’s calls or e-mail within 24 hours unless Coach is temporarily unavailable, in which case Coach will inform Client of the length of time Coach will be unavailable.

4. Client understands that Coach is a coach as defined in Section I above and not a professional therapist or physician. Coach is not trained in diagnosing mental or medical conditions and will make no attempt to do so. Should issues arise that should be handled by a licensed therapist or physician, Coach will require that Client attend to these through the aid of the appropriate professional.

5. Coach will bring attentive listening, understanding and commitment to Client’s growth, and a firm belief in Client’s ability to affect the changes Client desires.

6. Should there arise, for any reason, doubt that Coach is the best resource for helping Client achieve his/her goals, Coach will inform Client of this decision and suggest a more appropriate coach.

7. Client can expect Coach to ask questions, offer fresh perspectives, make requests (including assigning homework), acknowledge wins, challenge limiting beliefs, and guarantee confidentiality in a supportive and respectful manner.

Client

1. Client brings motivation and commitment to the coaching relationship.

2. Client agrees to contact Coach within 24 hours of the appointed coaching session if a cancellation needs to be made. Client agrees to pay for any missed sessions if no contact has been made and/or if the Client does not show up for the appointed session.

3. Client agrees to take the appropriate actions to achieve the personal and professional goals for which the coaching relationship is intended.

4. Client realizes that anything less than full and intentional participation will not result in the level of progress desired and that is possible with full and focused participation.

5. Client accepts full responsibility for self, actions and outcomes.

6. Client agrees not to hold Coach responsible or liable for the choices Client makes or for their resultant affect.

7. Client has read and agrees with the coaching terms and conditions of this Coaching Agreement.

SECTION IV: Cancellation of Agreement

1. Either party may cancel this agreement at any time with or without cause by informing the other party in writing that the agreement has been terminated.

SECTION V: Schedule

1. The parties will meet at: a location to be defined when booking appointment

2. For an agreed upon frequency and duration.

3. The specific dates for said meetings will be agreed upon prior to the first session and the coach will send a calendar invite to the Client.

4. Client agrees to pay per session. CORE Map (has/has not) been added to the coaching package at a discounted rated of $50 add-on to the coaching session fee. Client also agrees to pay for services via check or credit card per agreed upon payment schedule.
